To assess for a tear or significant weakness in the **subscapularis tendon** (subscapularis strength).

How do I start?

Patient is sitting or standing. The affected hand is placed on the opposite shoulder, with the elbow pointing forward.

What happens?

The patient actively pushes their hand into their shoulder. The examiner attempts to **externally rotate** the patient's arm and pull the hand away from the shoulder while the patient resists.

In Plain English

What Does a Positive Result Mean?

The patient is **unable to hold the hand against the opposite shoulder** due to weakness or pain, indicating a subscapularis tear.
