To evaluate the integrity and tightness of the **posterior shoulder capsule** and diagnose **posterior instability**.

Watch How It's Done

How do I start?

Patient is seated. The arm is in 90° abduction and maximal external rotation.

What happens?

The examiner applies an **axial load** to the humerus while simultaneously **adducting the arm across the patient's chest** (horizontal adduction) and maintaining external rotation.

In Plain English

What Does a Positive Result Mean?

The patient experiences **posterior shoulder pain or apprehension**, and/or the examiner feels **excessive posterior translation** of the humeral head on the glenoid.

Helpful Tip:

This test puts a strong stress on the posterior capsule.
