Understanding the Painful Arc Assessment

To diagnose **subacromial impingement syndrome (SAIS)**, specifically by identifying compression of the rotator cuff under the acromion.

Watch How It's Done

How do I start?

Patient is sitting or standing with the arm resting at their side.

What happens?

The patient is asked to **actively abduct their arm** from a neutral position to full elevation.

In Plain English

What Does a Positive Result Mean?

Pain occurs within the specific range of **70° to 120° of abduction**, but pain is significantly diminished or absent both below 70° and above 120°.

Helpful Tip:

Pain occurring near end-range elevation (170°+) may indicate Acromioclavicular (AC) joint pathology.
